Required documents for Duplicate License:
1. Required Duplicate Application Form within FileCover
2. A copy of N.I.C
3. 2 Attested fresh Photographs (Passport size).
4. Report from Computer section for Computerized License or A certificate issued by relevant Post Office which expresses the expiry date of license.
5. FIR (Lost License report ) of the Police Station.
6. Tickets will be pasted according to the schedule.

Following documents may be submitted in relevant office:
1. Endorsement Form
2. A Copy of Medical Certificate.
3. A Copy of N.I.C.
4. 2 Attested Fresh Photographs.
5. Original Learner Permit of atleast Six Weeks.
6. Driving Tickets According to Schedule.
7. 1st license will be attached.
